DRESS REQUIREMENTS / RULES
The instructor may remove you
from a clinical experience if the following requirements are not met:
1. Scrubs will be worn.
They must be red and wrinkle-free. The Uniform Shop will be here August
26th and will bring sample scrubs for you to try on. She will sell them
for $25.75 for a pair. She will make out an order slip and you must have
her money ready the following week when she delivers them.
2. Your scrub top must identify
you as a student of CHS. Cinnamon Blanchard will embroider on your top if
you want or you can find someone or you can get a name tag.
3. Shoes are to be white.
Shoes must be kept clean.
4. Wedding/engagement rings may
be worn. Hoop earrings, necklaces, wrist and ankle bracelets cannot be
worn.
5. Hair must be clean, neat and
off the shoulders. Males should be clean shaved.
6. Do not wear cologne or perfume
on clinical days.
7. The wearing of facial
cosmetics should be moderate. No loud colors of eye shadow should be used.
8. Nails should be trimmed and
neat. Wearing long (false) nails will result in a missed clinical day.
9. You must have a waterproof
watch with a second hand and stethoscopes are optional.
10. You must act in a
mature and responsible manner and report any incident that occurs immediately.
11. If you are on free or reduced
lunches, financial help is possibly available for scrubs. Please notify me
if there is a need.
CLASSROOM INFORMATION / RULES
1. If you are injured in the
classroom or at the clinical site, you must inform the instructor immediately
and an incident form will be completed.
2. Parents must sign the rules
and dress code agreement.
3. You must bring your book,
handouts, paper, and pen to class daily.
4. Sleeping in class will not be
permitted.
5. All patient communication and
records will be kept confidential. A student will be removed from this
class if there is ANY violation of patient confidentiality. This is
a legal matter and must be adhered to strictly.
6. Attendance is a must.
Please call me or let me know ahead of time if you are going to be absent at
322-7782 or 532-7391 ext. 1048.
7. If you are pregnant or suspect
you are pregnant, please notify the instructor. You must bring a doctor's
statement that you can lift and perform nurse aide duties.
8. Participation in class
discussion, role-playing, demonstrations, and having a positive attitude is
expected in class.
9. You will be sent to In-School
Supervision if you are not ready to go to clinicals. You will have papers
to complete to obtain your points.
10. You will need a T.B. skin
test and the Hepatitis B vaccination series (3 injections).
11. A criminal background
check will be done on all applicants who take the certified nurse assistant
exam. The fee is $15.00.
12. You will receive a copy
of the Health Care Worker Background Check. If you have a bad
background check you can still take the exam, but may not be able to obtain
employment in a direct care position.
The exam will be at Kaskaskia
College. The fee for taking the exam is $50.00.
You must demonstrate skill in
performing all 21 of these tasks in order to successfully complete the training
program.

It is important that you
understand the nature of this class and the requirements mandated by the
Illinois Department of Public Health as follows:
**Successful completion of
21 mandated skills
Completion of at least 40
hours of clinical
** Incur no more than 10
absences in the first three quarters.
** A deficiency in any one
area would render the student ineligible for certification.
